Hold Harmless Agreement (HHA)
As a participant in any class offered by Danielle’s Yoga and Dance Space, I am aware that any physical activity involves the potential of risk, including from infectious diseases/COVID-19. In consideration of above participants being permitted to participate I agree to assume full responsibility for any risks, injuries and damages which I might incur as a result of participating in any class offered by Danielle’s Yoga and Dance Space which may be sustained by undersigned.
I agree to the Hold Harmless Agreement listed above.
Kids Creative Movement
Policy and Procedures:
-Class will be held May 3, 10, 17, 24 and June 7 from 4:15-5:00pm
-No class on May 31 in observance of Memorial Day
-Class is $65 for the 5 -week session and will be paid before the first day of class to reserve your child’s spot
-In the event of weather cancellation, parents will be notified at least one hour before class starts.
-Make-up dates due to weather will be held on Wednesdays from 4:15-5:00 pm
-To ensure the safety of teacher and students, children and teacher are required to wear masks. Social distancing will also be practiced to the best of our ability.
-Please bring children in comfortable, weather appropriate clothing with sneakers on their feet and no jewelry
-Child will not attend dance class if they are experiencing symptoms of Covid-19 or have come in close contact with someone who has tested positive for Covid-19.
